The Incident

A man in his twenties was on suicide watch at Chicago Lake Shore Hospital. While he was there, based upon the “watch” status, the staff was required to regularly observe him and be mindful of any rash actions he might take to attempt to end his life. It was clear that he was able to take his life due to the failure of the staff to execute their observation duties.
